Job Description
Position Summary
The Pediatric Care Navigator will participate in a dynamic interdisciplinary team to provide diversified support to providers and clinical team members as well as patients/families. The Care Navigator will partner with patients and families by engaging them in the accountability of care in order to improve patient experience. The Care Navigator will develop effective partnerships with patients/families, staff and systems to create an environment of high quality and patient-focused outcomes with an emphasis on coordinating care for patients identified with complex medical needs. Coordination will include direct communication with external specialists and resources, thorough follow up with families, and help with navigation of complex medical systems.
Essential Job functions
  • Works with multidisciplinary transitional care team to facilitate seamless patient transitions across the continuum of care.
  • Independently prioritizes care management needs.
  • Documents appropriately in the electronic health record, and any required patient tracking documents. Employs appropriate and timely use of tasking in the EHR. Reviews and updates medications.
  • Performs all care management activities across the continuum of care while adhering to the core values of patient confidentiality, privacy, safety, advocacy, and adhering to ethical, legal, and accreditation/regulatory standards.
  • Works through direction of providers to ensure quality of care. Provides direct patient care in department, including but not limited to patient rooming and vaccination administration following Summit Health protocols.
  • Other duties as assigned.
General Job functions:
  • Coordinates referrals to specialists and community resources
  • Supports families with travel coordination for out-of-area services
  • Connects patients with medical equipment needs, such as diabetic supplies
  • Processes medication pre-authorizations
  • Connects patients with ADHD to appropriate resources.
  • Coordinates behavioral health support.
  • Direct patient care within scope of medical assistant job functions.
  • Provides ongoing monitoring of patient involvement, participation and compliance with treatment plan, updating Care Plan as needed.
  • Demonstrates ability to use the EHR, and all ancillary clinical systems efficiently and effectively for communication and documentation purposes.
Education, Certification, Computer and Training Requirements:
  • 3-5 years of experience in Medical Assisting and certification required
  • Previous care Navigator or care navigator experience preferred
  • High School or GED equivalent
  • Valid Driver's License required
  • Demonstrated ability to work in a collaborative team-oriented environment
  • Ability to communicate in English, both orally and in writing required.
  • Proficient Microsoft Office skills preferred
  • Experience with standard office equipment (phone, fax, copy machine, scanner, email/voice mail) required.
