We're looking for someone to join our team as a AFS Case Manager (AFLP) who:
The AFS AFLP Case Manager is responsible for coordinating services for the program's target population as required by the Scope of Work and other contract requirements. The AFLP case manager will provide supportive services to youth clients, act as a liaison between the client and service providers, provide information and referrals to appropriate resources, and perform advocacy services. It is the overall task of the AFLP case manager to implement the program as the funder requires, help coordinate the client's comprehensive youth development, health, nutritional, educational, psychosocial, job training, and related support services, and educate clients on becoming independent consumers of the service delivery system.
Essential Functions:
  • Responsible for conducting an intake and all requirements for program enrollment.
  • Responsible for completing comprehensive assessments in multiple areas, such as health, nutrition, psychosocial needs, education/vocation, goals, parenting skills, and infant/home safety for each client enrolled in the AFLP program.
  • Responsible for follow up and completing appropriate releases of information to ensure client compliance with their individual service plan.
  • Advocates for clients when there is a problem in the service delivery system.
  • Provides guidance and support for clients and families.
  • Responsible for maintaining assigned case load and client contacts as required by Scope of Work, contract requirements and/or program protocols.
  • Keeps accurate, up-to-date records on clients served including contact summaries, progress notes, support service plans, and any program-specific reports.
  • Prepares and delivers presentations to the public about the AFLP program.
  • Works with other staff to develop community resources.
  • Serves as liaison with other community agencies and schools.
  • Reasonable and predictable in person attendance.
  • Other duties as assigned.

You'll be successful with the following qualifications:
  1. B.A. or B.S., in a health, social science or related field is strongly recommended.
  2. Years of case management experience, preferably with a youth population, may be substituted for the bachelor's degree. As such, an associate's degree will be the minimum education required.
  3. Experience working with youth target population is strongly recommended.
  4. Familiarity with community resources is strongly recommended.
  5. Ability to work effectively in an interdisciplinary team approach with both paraprofessionals and professionals.
  6. Ability to maintain patient/client dignity and confidentiality.
  7. Bilingual (Spanish-English) may be required depending on the need at the time of hire.
  8. Flexibility to alter the 8:00-5:00 Monday through Friday work schedule to meet program needs.
  9. Exceptional documentation and verbal skills are required.
