Our company, Gentiva Hospice, is seeking a compassionate and experienced Hospice Patient Care Manager to join our team. As a Patient Care Manager, you will oversee the day-to-day operations of our hospice site, ensuring the delivery of high-quality care to our patients and their families. In addition to managing clinical and administrative functions, you will also provide direct skilled bedside nursing care in patients' homes and participate in the on-call nursing schedule. This role requires a Registered Nurse with a minimum of an Associate's Degree in Nursing, along with at least three years of direct patient care experience and one year of management/supervisory experience.
