New full time Case Manager opportunity with Unity House of Troy!$750 sign on bonusUnity House is opening a brand new six-bed Crisis Residence that offers a safe place for the stabilization of psychiatric symptoms and a range of person centered, trauma informed services to support participants.This is a short term stay facility that serves participants for up to 28 days as they prepare to transition back to the community.Two positions are available one with a primary schedule of 8-4 and one with a primary schedule of 2-10. Both positions include one weekend work day which may have different hours.Position available October 21, 2024.Responsibilities:Provide crisis intervention and case management servicesProvide encouragement and positive feedbackAssist with implementation of service plansHelp develop Wellness Recovery Action Plan and Crisis PlanProvide accompaniment, support and advocacy as neededAssist participants in building life skillsMake referrals to local services and other resources to support participant goalsParticipate in rotating on-call systemPromote a culture of inclusion and belongingRequirements:High School diploma or GED required and at least 1 yr of experience working with adults living with a mental illness. Valid NYS Driver License. Reliable transportation. Excellent verbal and written communication skills. Computer skills.
