Position Summary

The Brookline High bryt Clinical Coordinator is primarily responsible for assisting high-risk adolescents in their reentry back to school and community after extended mental health/health-related absences which includes outreach and support to adolescents and their families, development and implementation of transition plans, consulting with the Brookline High School staff, local agencies and hospitals in the community, crisis intervention and participation in the evaluation and program planning.

Key Responsibilities

  • Operates and continuously improves an intensive intervention dedicated to successful transition back to school for students who have experienced significant mental health challenges and have missed extensive amounts of school time.
  • Develops and ensures effective implementation of a comprehensive transition plan integrating clinical, care coordination, academic, and family supports for each student in bryt; has primary responsibility for implementation of the clinical, care coordination, and family support components of the overall plan.
  • Organizes and facilitates meetings with staff of collateral agencies prior to and during students’ participation in bryt.
  • Provides 1:1 and/or small-group clinical intervention to bryt students on a flexible, as-needed basis, employing clinical methods that are most appropriate for meeting the goals in each student’s case management plan.
  • Proactively communicates with educator colleagues and school administrators regarding the progress and status of each student in bryt, maintaining student confidentiality as appropriate.
  • Engages in consistent two-way communication with the families of bryt students to keep them apprised of student progress and gather feedback and information to support student learning and well-being, including making home visits to families where appropriate; develops and provides family support and engagement activities.
  • Maintains a program environment that is conducive to learning and appropriate to the maturity and interests of students.
  • Provides functional support/supervision to assigned MSW social work intern in the bryt program.
  • Ensures that all required data is gathered and entered into data management systems according to program procedures.
  • Develops reasonable rules of classroom behavior and procedure, and maintains order in the classroom, including addressing inappropriate student behaviors as needed.
  • Other duties as assigned in relation to function of the bryt program.


Education and/or Experience

Master's level degree or above, with independent license preferred - LICSW, LMHC, LMFT, PhD/Psyd, MD

2+ years direct clinical care with children/adolescents

Experience working in school-based setting preferred

Bilingual/Bicultural candidates strongly preferred


Qualifications

  • Strong clinical skills demonstrated in an outpatient or school setting.
  • Excellent case management skills.
  • Ability to coordinate services across systems.
  • Excellent communication skills (verbal and written).
  • Proficiency with documentation in an electronic health record.
  • Proven ability to collaborate with providers, families, and social service agencies.
  • Desire to develop and maintain collaborative relationships.
  • Excellent organizational skills.
  • Demonstrated commitment to, and experience working toward, diversity, equity, and inclusion
  • Ability to manage some level of change and growth.
