Goddard is a partner within the Manhattan Outreach Consortium (MOC) a borough-wide initiative to serve the unsheltered community. MOC collaborates with various community partners and healthcare agencies throughout Manhattan.Purpose of Position: The Senior Housing Outreach Specialist works creatively and effectively in the community to engage individuals living on the street through outreach by utilizing a harm reduction approach and acting as a wealth of knowledge regarding harm reduction practices on the team. The position also serves as a case manager for adults experiencing street homelessness, with the goal of placing them into transitional and permanent housing. The position works collaboratively across teams and organizations to achieve this goal.Schedule:Monday through Friday 9:00 am to 5:00 pmSalary:$41,987Roles, Responsibilities, and Essential DutiesHarm ReductionPerform targeted joint outreach and engagement in areas known to have high substance us within the unsheltered populationProvide harm reduction supplies, education, and resources to unsheltered clientsAssist with team trainings in harm reduction practices and serve as the Harm Reduction Champion for the teamWork as part of an inter-disciplinary team to promote wellnessMaintain an active resource directory of harm reduction resources in catchment area and act as liaison with these resources and the teamCase ManagementServe as case manager for homeless adults who use substances, with a goal of placing them in transitional and permanent housingPerform intake, assessment, service planning, advocacy, referral and escort servicesMaintain client case records and enter data in computer database using handheld and desktop computersProvide follow-up case management for clients placed in housing, to promote retention and document outcomesAttend staff meetings, case conferences, trainings and supervision meetings; participate in program evaluation and improvementCommunicate with evening/weekend teams and with staff of other agencies to coordinate client servicesParticipate in outreach trips to identify and engage with homeless clientsDrive outreach vehicles and use public transportation as necessaryParticipate in the annual HOPE count and quarterly counts of homeless individuals living on the street, 4 overnight counts completed annuallyAssist with onboarding of new Housing Outreach SpecialistsOther duties as assignedQualifications/Educational RequirementsBachelor’s Degree preferredPrior experience with the homeless , substance use, or mental health preferredValid driver’s license and a driving record that meets the agency’s auto insurance guidelines (including at least one year as a licensed driver and no suspensions within the last 3 years)Flexibility in work hours; available to work evening, overnight and weekend shifts when requiredSkills, Knowledge, and AbilitiesExperience in Harm Reduction preferredAbility to be flexible in a fast-paced work environmentStrong written and verbal skillsTeam playerGood judgementComputer Skills: To perform this job successfully, an individual should have/be:Able to work in the Microsoft Office Suite, should especially be proficient in Word and OutlookAble to use or learn other electronic databases, programs, and funder-related software, such as Mocapp, Streetsmart, HMIS, and AWARDSPhysical RequirementsFrequent walking and standing,Ability to lift up to 30lbs such as client belongings during movesWork EnvironmentIndoors and outdoors; all weatherWe are committed to building an inclusive workplace community grounded in respect for differences.
