ResponsibilitiesPrimarily responsible for providing treatment and education to patients and ACTION team on drug trends, substance use disorders, and dual diagnosis substance abuse services.
Provides individual and group SUD treatment.
Coordinates with treatment programs to assist patients with residential/outpatient programming, coordinating assessments and detoxing treatment.
Participates in the ongoing assessment of symptoms and response to treatment.
Consults with community agencies and families to maintain coordination in the treatment process.
Initiates and maintains relationships, in coordination with other staff, law enforcement, human service agencies, & informal community resources, (e.g. landlords, employers, etc.).
Participates in patient-centered comprehensive assessment
Be accessible by telephone and prepared to respond to calls during all scheduled shifts within 60 minutes.
Develop & maintain working relationships with other mental health practitioners, case managers, law enforcement, school personnel, hospital personnel, and other interested parties.
Provide crisis assessment and will take necessary time with client to determine course of action.
Complete a thorough safety plan with clients if applicable.
Provide follow up within 24 hours of end of crisis interaction.
Documents patient progress to maintain a permanent record of activity according to established methods and procedure and in a timely manner
Maintain compliance with Human Resources policies and procedures.
A team member provides support, advocacy, coordination, problem solving, direct assistance, training, and supervision to help patients obtain necessities of daily living.
Functions as a practicing clinician on the team and provides service coordination (case management) for an assigned group of patients.
Coordinates and monitors activities of daily living.
Assumes primary responsibility for developing, writing, implementing, evaluation and revising treatment goals and plans in conjunction with client.
Provides supportive therapy and symptom management.
Ensures immediate changes are made in treatment plans as a patients' needs change.
Educates and supports patients' families as needed.
Advocates for patients' rights and preferences.
Help patient maintain entitlements.
Consults with community agencies and families to maintain coordination in treatment.
QualificationsEducation:
Associates of Science or Associated of Arts, High school diploma or GED, CADC and minimum of 2 years experiences within substance/mental health field.
Bachelor's in human services, Psychology, Social work or related fields
Experience:
Experiences in individual and group substance use disorder and Co-Occurring treatment.
License(s)/Certification(s):
Valid driver's license, must obtain a class D license.
Must have CADC, IADC, IAADC
Knowledge/Skills/Abilities:
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adolescents, adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills. Position also requires a valid driver's license.
