This is a multifaceted position with a range of dynamic projects and responsibilities which serve to assist the Director of Population Health. This individual should be capable of effectively completing high-level management projects, as well as fulfilling coordinator, scheduling, administrative, and analytical duties.
Essential Duties and Responsibilities include the following:
  • Fulfills coordinator duties including scheduling, documentation, calling members, auditing and assisting program managers during staffing shortages.
  • Works collaboratively with Population Health department staff to produce reports detailing program performance.
  • Develops, updates and maintains department documentation such as job aids, intake forms, marketing materials, evaluation materials, audit forms, assessments, data collection logs, and other materials and projects as needed.
  • Ability to stay organized, meet company and departmental goals, and submit timely and high-quality deliverables to the Director of Population Health.
  • Works cooperatively and collaboratively with the executive team as well as medical directors.
  • Monitors and analyzes programs and interventions to improve processes and outcomes.
  • Identifies opportunities for the development and implementation of new approaches that improve efficiencies and production while maintaining quality.
  • Performs all other duties as directed by management.
The pay range for this position at commencement of employment is expected to be between $21 - $23 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements
Education and / or Experience:
  • Bachelor’s or associates degree preferred
  • Bilingual strongly preferred.
  • At least 2 years prior full-time work experience (healthcare not required, but preferred).
  • Extremely strong verbal and written skill set. Ability to write high-level communications to executives.
  • Strong quantitative skill set, including basic statistical knowledge, and intermediate to advanced mathematics knowledge.
  • Ability to utilize advanced Microsoft Office Skills in projects, including Microsoft Excel graphing features, Microsoft Vizio flow charts, and Microsoft Word.
  • Ability to perform any administrative duties such as scheduling, preparing training rooms, ordering team lunches, arranging group outings, and attending to other management needs.
