Behavioral Health Care Manager - Remote
DIRECT ACCESS is looking for a bilingual medical assistant to provide remote care management support to patients that are primarily home bound.
Responsibilities include:

• Coordinate care needs for patients.
• Communicate with patient's care team, which may include physicians, family members, and other care providers.
• Document care management communications.
• Create and maintain a comprehensive, patient centered care plan.
• Utilize the care plan to assist patient with achieving their goals.
• Provide education for chronic and specialty disease conditions.
• Other duties as needed.
