Residential Services Case MangerBoston, MA ( • HousingJob TypeFull-timeDescriptionSalary Range: $48,450 - $53,295 per yearSchedule: Monday – Friday from 8:00 am – 4:00 pm.Job Summary:The Residential Services Case Manager provides direct support to formerly homeless individuals who have transitioned into permanent housing. They operate from a “housing first” point of view, recognizing that the main goal is to work closely with residents to ensure they have the support to develop and apply skills and access to resources they need to live successfully in stable permanent housingThe Residential Services Case Manager uses the principles of the St. Francis House Philosophy of Care as a guide for his/her work (being person-centered, recovery-oriented, and trauma-informed) and keeps in mind the goals of helping individuals achieve outcomes in the areas of housing, income and behavioral health stability.Essential Duties/Responsibilities:Welcomes new residents into housing and provide services and supports that foster a smooth transition from homelessness to a stable living environment in permanent supportive housing.Carries a caseload of approximately 30 individuals, whose needs will vary over time, but approximately 30% of which will have high levels of service needs at any given time.Uses a proactive approach in developing positive relationships with residents, reaching out on a regular basis with all individuals on the caseload to ensure they know what, where, and how to engage in services when needed.Conducts assessments and builds collaborative service plans with residents, prioritizing areas that are directly connected to improving levels of stability in housing, income, and behavioral health, establishing agreed-upon goals.Works with participants to develop and or strengthen daily living skills including budgeting, employment/income, food and nutrition, and good tenancy practices.Understands and respects individuals’ choices and levels of readiness to change and uses Motivational Interviewing to provide encouragement, insight, and informed support.Works collaboratively with the other members of the housing team and community to develop, promote, and facilitate positive activities for residents living in St. Francis House housing communities.Integrates with other departments within St. Francis House to coordinate consultation and care for residents that address behavioral health, income, and refuge services.Maintains ongoing and up-to-date resident records in the database (Clarity and Google Docs) and regularly reviews caseload activities and goals.Monitors participant’s tenancy to identify any potential concerns early, including landlord-tenant disputes, rent arrearages, and lease or code violations (by tenant and/or by landlord). Reports out any concerns to supervisor promptly.Serves as a liaison with participant’s landlord, property management, medical and behavioral health providers, and others as needed and permitted by participant.Self-monitors performance to work with supervisor to insure measurable progress with participants and adherence to contract requirements, departmental goals, and/or agency-wide outcomes.Adheres to agency code of conduct.Performs other duties as assigned.RequirementsMinimum requirements: High School Diploma or equivalent required. Further education in a human service field a plus.Experience with case management/direct service with vulnerable populationsMinimum of 1-year experience working directly with people experiencing or who have recently exited homelessness preferred.Lived experience with homelessness and/or recovery a plus.Driver’s license and regular access to reliable transportation strongly preferred.Lived experience of homelessness and/or recovery a plus.Bilingual (Spanish), preferred.SFH is an Equal Employment Opportunity Employer is committed to a diverse and inclusive workforce where all staff can reach their fullest potential.
