We are seeking a Health Care Coordinator for our Home and Community Based Services Wait List program. In this role, you will be responsible for developing individualized care strategies and coordinating services to ensure timely and cost-effective support for individuals with long-term care needs. This includes advocating for individuals, conducting home visits, and serving as a resource for community care coordinators.
Location: Northeast KS - Johnson, Douglas, Wyandotte, Atchison counties.
Working Schedule: Monday through Friday, flexible hours.
Key Responsibilities:
- Assess, plan, and implement individualized care strategies
- Initiate referrals for social service programs
- Advocate for individuals and families to ensure their needs are supported
- Coordinate with care team and conduct in-person visits as necessary
Required Qualifications:
- Bachelor's Degree in a related human services area
- 1+ years of experience with long-term care needs
- Intermediate level of computer proficiency
- Ability to travel 75% of the time for the role
- Valid driver's license and access to reliable transportation
Preferred Qualifications:
- Licensed Social Worker or clinical degree
- Experience with managing complex medical or behavioral needs
