Job Description
The candidate will work closely with the Restoration Services Manager and Hospital-Based Interventionist to support families affected by gun violence. This involves assisting in the development of a hospital discharge plan and conducting safety assessments with the family. The role requires maintaining regular contact with families, facilitating their connection with community and public aid resources, and participating in the organization of vigils and memorial services.
Key Responsibilities:
Initial Support and Planning: Collaborate with the Restoration Services Manager, Hospital-Based Interventionist, and the family to formulate a hospital discharge plan. Conduct a comprehensive safety assessment for the family.
Family Engagement:
  • Phase One (First 72 Hours): Implement safety planning and hospital discharge planning with the family, the Community Safety Team, and Hospital-Based Interventionist.
  • Phase Two (2-4 Weeks): Ensure daily communication and at least two in-person meetings per week.
  • Phase Three (2-3 Months): Maintain communication every other day and organize weekly in-person contact.
Community and Public Aid Resources: Link families with community-based well-being and mental health resources, and assist them in accessing public aid such as Medicaid, SNAP, TANF, and victim services.
Event Support: Collaborate with the Restoration Services Manager to assist in organizing vigils, funerals, and memorial services as required.
Team Collaboration: Participate in meetings with the Care Team for case reviews and attend regular supervision meetings with the Restoration Services Coordinator to discuss workload, achievements, and challenges.
Professional Development: Attend all mandatory meetings, training sessions, and professional development opportunities.
