The Salvation Army Ferguson Community Empowerment Center in Ferguson is hiring for a Pathway to Health Case Manager. This individual will recruit, enroll, and apply appropriate intervention for clients in the Pathway of Hope and Pathway to Health programs. Position is community based with frequent local travel.

This is a full-time, 40 hours per week, $20.19 per hour

Essential Duties :

Pathway services consist of the following functions:

  1. Recruitment:

  • Conduct recruitment activities to identify potential participants in the Pathway of Hope and Pathway to Health programs.
  • Build relationships with outside agencies for regular client referrals.

  • Interventions
    • Conduct pre-screening and intake of clients as necessary using specified assessment tools.
    • Conduct goal setting steps with clients formulating change-oriented action plan.
    • Conduct follow-up case management meetings with clients.
    • Monitor and track changes in goal attainment per the action plan.
    • Conduct Home Visits regularly with Pathway families
    • Distribute vouchers, and/or checks as available to clients for purchases and payments and refer persons to other agencies if assistance is unavailable.

  • Documentation & Spending
    • Record electronic and written client information in SIMS & HMIS databases within 48 hours.
    • Record statistical data and provide statistical reports in a timely manner.
    • Case Manager will expend funding and in-kind gifts in a manner that conforms to spending deadlines and utilizes tracking methods prescribed by Salvation Army policy. All reports must be submitted in a timely manner.

  • Outreach
    • Network with community agencies and organizations to maximize services to persons and occasionally liaison between clients, The Salvation Army, and other agencies.
    • Identify resources available in-service areas and maintain a current community resource guide to provide information and referral services and inform persons of the resources available to them.

  • Other
    • Participate in seasonal events as directed by supervisor.
    • Provide emergency social services during disaster situations for extended durations and in extreme conditions.
    • Represent The Salvation Army through the implementation of its mission in all service aspects.
    • Maintain confidentiality and comply with Safe From Harm guidelines.

    • Maintain a positive work atmosphere by behaving and communicating in a manner that fosters good relationships with clients, co-workers, and supervisors

    Minimum Qualifications: Bachelor's degree in human services field (BSW/MSW preferred) with one (1) year related work experience. Must have an outgoing personality with good communication skills and an ability to work with diverse cultures. Strong organizational skills required. Commitment to carry out The Salvation Army's mission is essential. Knowledge of The Salvation Army and Government, State and community resources is helpful. Must have working knowledge of computers.

    Motor Vehicle Requirements: Valid drivers' license and pass TSA MVR check. Travel : 70% community-based; 30% Office-based. Must have reliable transportation for local travel.

    Physical and Work Environment Requirements : Must be capable of performing the above essential duties in an office environment to include using a computer, answering phones, etc. Most work will be indoors in a temperature-controlled environment. Some duties involve leaving the headquarter site to attend meetings.
