What You'll Do
Position Summary
The Manager, Care Management will work with our multidisciplinary care team to provide high quality care for our high-risk patients within Suvida Healthcare. The manager will collaborate with the clinical operations and multidisciplinary care team to develop organization-wide approaches to problem solving, tracking, and managing complex cases and populations. This leader will identify and analyze current systems and variances to identify opportunities for improvement. The manager will hire, train, and develop a care management team and create care pathways for the team to follow for various chronic conditions. Identifies community resources for patients and will transitions of care for patients receiving post-acute care. Works to promote quality of care through collaboration with all providers, care team, patients, and families. The manager will oversee a care management team that cares for patients who are highly complex and resource intensive within Suvida Healthcare. This leader will need to plan effectively to meet patient needs, identify social determinates of health, manage chronic conditions, and promote efficient utilization of resources. Essential responsibilities consist of but are not all inclusive:
Responsibilities
  • Strategically lead the Transitions of Care, Chronic Care Management, and Triage departments, ensuring seamless, efficient, and compassionate service delivery.
  • Expand the scope of Nurse Care Management to include initiatives like preventive care programs, health promotion and education, and community outreach, ensuring these programs align with patient needs and organizational goals.
  • Spearhead the development and implementation of nurse-led initiatives that enhance patient care, such as shared decision-making, health literacy improvement, and patient engagement strategies.
  • Foster a culture of excellence, continuous learning, and patient-centered care among the nursing teams, facilitating advanced training programs, peer reviews, and team-building activities.
  • Collaborate with interdisciplinary teams to refine clinical pathways, integrating innovative care models and evidence-based practices that enhance patient outcomes across various departments and care settings.
  • Serve as a key executive stakeholder, actively engaging with healthcare professionals, payors, regulatory bodies, and community representatives, ensuring Suvida's care standards and initiatives are well-communicated and supported.
  • Drive quality and performance improvement projects, utilizing data-driven approaches to advance patient satisfaction, health outcomes, and operational efficiencies while reducing healthcare disparities.
  • Ensure fiscal responsibility across all care management departments, optimizing resource allocation, and demonstrating value in patient care initiatives.
  • Advocate for the integration of advanced technology and digital health solutions to improve patient access, monitoring, and ongoing health management, especially for those with chronic conditions.

What You'll Bring
Knowledge, Skills, and Abilities
  • Master's degree in Nursing, Healthcare Administration, or related field preferred.
  • Demonstrated expertise in managing comprehensive healthcare functions, with a special focus on Transitions of Care, Chronic Care Management, and Triage.
  • Experience in developing and leading nurse-driven initiatives that enhance patient care, staff proficiency, and system-wide health outcomes.
  • Proficiency in analyzing complex healthcare data to inform strategic decisions, coupled with an understanding of current healthcare economics.
  • In-depth knowledge of regulations, accreditation standards, and ethical mandates in healthcare.
  • Exceptional leadership qualities, including the ability to motivate, inspire, and drive multidisciplinary teams toward a common vision.
  • Strong communication and interpersonal skills, capable of effectively engaging with diverse stakeholders and advocating for healthcare innovation.
  • Capacity for high-level strategic thinking, problem-solving, and decision-making.
  • Bilingual proficiency in English and Spanish (strongly preferred).
  • Proficiency in Microsoft Office platforms, including Word, Excel, and PowerPoint
Education, Experience, Licensure, or Certification Requirements
  • Active Registered Nurse (RN) license, with 5+ years of progressive experience in clinical care, nurse management, and healthcare leadership.
