Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.Job DescriptionThe Manager of Case Management is responsible to oversee the day-to-day operations of case management and care coordination and supervises all case managers, within the Recuperative Care Department for Los Angeles County. The primary tasks and responsibilities include supervision of staff, assisting in the monitoring of budgets, developing and implementing policies and procedures. The Manager of Case Management will oversee aspects of client care and will link homeless clients with needed available resources, monitor clients’ progress towards goals and provide comprehensive services empowering clients towards self-sufficiency.The salary range offered for this position is $66,560 - $70,000 per annum, depending on experience.ResponsibilitiesSupervision:Supervise case management staff for daily tasks, case oversight, training, and growth.Coordinate hiring process of new case management staff.Work with Site Managers on all client decisions including exits, extensions, intakes, and other case planning action steps.Coordinates orientation and ongoing in-service education for staff to improve skills and client careClient Care:Provide case consultation and emergency management for case oversight.Provide management support to assist case managers and clients in creating crisis management plans or plans to support client’s progress towards self-sufficiency. Plans can include interventions for sobriety, safety, self-harm contracts, employment, housingretention, increase in income, and child safety.Oversee care coordination in partnership with Program Managers and Site Managers for all programs to ensure client care, effective case planning, and positive outcomes in line with the mission of the organization and contract outcomesDocumentation:Ensure accuracy of data for contract compliance, grant reports and other reporting needs.Assists Program Manager with contract compliance as requested.Collaboration:Attend community meetings and collaborations to build relationships and partnerships.Coordinate resource presentations for case managers to increase knowledge of community resources.Mission Support:Demonstrate a commitment to Illumination Foundation’s mission and core values, while developing and maintaining respect and harmony with all employees and management.Implements programs and policies designed to maximize the efficiency of program and ensure that all client needs are met.Must display full understanding of the problems of client flow and scheduling, in order to improve the efficiency of program operations through performance of duties.Must demonstrate the ability to motivate staff, grasp new skills/procedures and foster a positive attitude toward changes.Must be an asset to the organization and a positive influence on co-workers, while displaying respect toward patients and others in the workplaceActs in an ethical manner reflecting core values of integrity, transparency, accountability, respect and responsibility.
