Job Requirements / Description

The Outpatient Care Coordinator provides support to clients and employees, including direct contact with clients and their families in a number of roles. This also includes assisting in access to services, addressing needs/issues/concerns that arise during the course of treatment, as well as assisting with various documentation requirements, record keeping and case management items.
Care Coordination
* Process and coordinate referrals into the program, which include ongoing services for our clients as well as the discharge process.
* Gathers information related to client care
* Schedules regular meetings with parents and clinicians to assess individual's progress toward goals.
* Provides timely feedback to team/service providers about service needs
Administrative Management
* Conduct necessary intake and discharge administration
* Coordinates with billing department regarding changes of insurance eligibility and authorization for services.
* Partners with Managed Care Organization (MCO) as needed
* Create and maintain patient accounts by obtaining, recording, and updating personal and financial/insurance information/authorizations; within established deadlines and with accuracy.
* Coordinate individual's information to support /teams/department/vendor/resources.
* Ensures documentation/files are compiled in a timely manner. This includes, but is not limited to, client vouchers, progress notes, treatment plans, aftercare plans, case consults, discharge summaries, assessments, letters and other client related correspondence and authorizations.
* Compiles information to develop and deliver analyses in an easy understandable format using basic to intermediate Microsoft Office programs within appropriate timelines

The ideal candidate will possess the following qualifications:
* Minimum high school diploma, bachelor's degree preferred
* 1-3 years' experience in a similar role
* Bilingual (Spanish) preferred
