The candidate will be situated in the Emergency Department and occasionally on the inpatient wards, working with the nursing staff and physicians to help assist in the treatment and follow up on the above screening tests. The department has mental health and substance use advocates, for which the employee may also consult as necessary for holistic wrap around care for patients.
We are seeking a dedicated and compassionate Care Coordinator to join our Emergency Department team, specializing in providing support and assistance to patients with HIV, HCV, Syphilis, Chlamydia, or Gonorrhea. The ideal candidate will play a critical role in ensuring that patients receive comprehensive care and support during their visit to the emergency department and facilitate their journey through the healthcare system.

Required:
- Bachelor's degree in Nursing, Social Work, Public Health, or a related field.
- Current state licensure or certification, where applicable
Preferred:
- A master's degree
- 1-3 years Previous experience working in an emergency department, infectious disease care, or a similar healthcare setting.
