JOB DESCRIPTION
Housing & Stability Coordinator
The Housing & Stability Program works with DHS involved families experiencing housing crisis. While services are being provided, the Coordinator will work closely with a Caseworker from Department of Human Services (DHS) to provide; assistance with housing system navigation, assistance with emergency placement and access to the local shelter system. Services also include, but not limited to, assistance with overcoming housing barriers, household management and stability services. The Housing & Stability Coordinator works a flexible forty-hour workweek, which may include early morning, evenings and weekends. This position reports to the Child Welfare Advocacy Programs Manager. Self Enhancement, Inc. - Community and Family Programs (CFP) is located at; 2205 NE Columbia Blvd, Portland, OR 97211.
DUTIES & RESPONSIBILITIES INCLUDE:
  • Short-term intensive case management to provide emergency housing assistance and housing system navigation assistance.
  • Immediate assessment of housing needs & determine if needs & barriers are urgent (30 days) or less urgent (31-90 days).
  • Develop client specific plan to address specific housing needs and assist with mitigating barriers to permanent & stable housing.
  • Attend Child Safety and Family Decision Meetings, or other case planning discussions with DHS staff and referred families.
  • Assist clients with connection to community programs that will increase their capacity to rent.
  • Establish and utilize networks within the housing system to assist clients including; landlords, housing agencies, rental assistance programs, & shelter services.
  • Provide personal skills training to increase client capacity to maintain stable housing including; budgeting, employment readiness, and financial planning.
  • Create a collaborative problem-solving approach incorporating the use of extended family, neighbors, other community persons, and the provider, as resources.
  • Assist and accompany clients to access eligible benefits and community resources to increase their financial stability.
  • Prepare written weekly reports, reviewing progress of family in housing & stability services. Also prepare additional or specialized written reports, as requested, for the purposes of court hearings or specific case planning meetings.
  • Coordinate services with other social service agencies (i.e. job training, financial planning, etc.).
  • Keep thorough and up-to-date client records in accordance with agency file requirements.
  • Attend scheduled training sessions and other trainings geared toward professional growth. Attend agency staff meetings.
  • Provide a daily and weekly work schedule.
  • Accept other appropriate duties/responsibilities as assigned by management.
Qualifications
QUALIFICATIONS:
  • A Bachelor's Degree in Social Services or three years' experience working with families and children.
  • Must be a highly skilled individual with needs assessment and case management experience.
  • Knowledge of crisis, cognitive and behavioral interventions.
  • Must have knowledge of, and ability to navigate, the housing system in Multnomah County.
  • Must have the ability to immediately motivate and connect with parents successfully.
  • Must have excellent writing skills, be highly organized and self-motivated.
  • Must have driver's license, insurance and own reliable transportation.
