Outreach Specialist: Part Time
This is a 100% grant-funded position.

Responsibilities:
• This position will provide evidence-based HIV prevention, risk reduction education and capacity building to individuals and grassroots/community based organization (GR-CBO) partners to improve the GR-CBO
• Partner's capacity to implement social networking and other proven engagement strategies for HIV self-test kit distribution and PrEP education/uptake; supports the provision of referrals for essential and supportive
• Services, ensures linkage to care, (escort from a community-based location to Montefiore Health System for clinical follow-up), supports recruitment for MPP's Community Engagement Group, intervention activities, and condom distribution;
• Responsible for eligibility screenings, intake and data entry for services provided, aimed at the programmatic goal of improving the health of MSM, LGBTQ+ and high-risk individuals in the Bronx and NYC, not limited to other job responsibilities
Knowledge, Skills & Abilities:
• Excellent interpersonal, communication and oral skills / Excellent public speaking to make presentations
• Proficiency in engaging the community and patients / Ability to communicate health education messages
• Knowledge of computer programs and proficiency in Microsoft Word, Excel and Power Point (Will also use photocopier and fax machine, as needed)
• Willingness and ability to work a flexible schedule to meet the needs of the community
• Second language a plus
Hours: Monday - Friday 8:30a-5:00p
Education:
Bachelor's Degree
Experience:
• At least 1 year of experience in healthcare and/or care coordination and care management
• Knowledge of care coordination and local community resources
Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.
