**Job Description:**
- Participate in a 24/7 street homelessness solutions initiative, engaging with individuals on streets, subways, and parks.
- Conduct medical, mental health, and safety assessments for the homeless population.
- Encourage individuals living on the streets to seek shelter and utilize available services.
- Work collaboratively with crisis coordinators and outreach teams to assess mental health conditions.
- Determine if individuals pose a risk to themselves or others and if immediate care is needed.
- Coordinate with law enforcement for safe transportation of individuals in crisis to psychiatric emergency programs if necessary.
- Collaborate on discharge planning and continuum of care strategies.
- Provide medical attention as per legal guidelines and initiate relevant procedures if needed.
- Document encounters and assessments efficiently in the field or office setting.
- Communicate with hospital staff and clinicians to relay medical assessments and treatment recommendations.
- Obtain necessary approvals to operate as part of a mobile crisis outreach team.
- Maintain and update records promptly.
