Job Summary:
The Social Worker is responsible for assisting the Social Work Department in providing direct and indirect service and assistance to residents and families in matters affecting their psychosocial needs. Assists the Social Work Department in providing comprehensive social work services upon admission, during resident stays, and planning effective discharge plans upon admission. This individual serves as a member of the interdisciplinary team.
Duties/Responsibilities:
Pay Range:$25-$30/hour
Employee salary is based on commensurate experience and other qualifications.

Duties/Responsibilities:
- Assists with facility admissions, including completing intake/ admission paperwork with resident/ family.
- Provides facility tours for prospective admissions.
- Coordinates and assists with room transfers.
- Assists with resident interviews and evaluations of the resident's biopsychosocial needs to aid development and implementation of resident care plans.
- Coordinates discharges and related referrals to ensure successful transition to the community.
- Participates as an active member of the interdisciplinary team to assist in promoting the overall well-being, physical, mental, psychosocial needs of the residents.
- Maintains written documentation in the medical record per facility policy and state and federal guidelines.
- Offers support and referrals, as necessary, to help residents cope with the social and psychological aspects of their illnesses, impairments, addictions, or disabilities as well as their feelings about separation from family/community, physical and emotional loss-including trauma-induced events.
- Assists in implementing social services policies and procedures that conform to current standards of practice, facility philosophy and operational policies, while maintaining compliance with state and federal laws and regulations.
- Occasional community outreach as needed to promote and educate regarding the facility and its services.
- Promotes customer service and hospitality and collaborates with interdisciplinary team, resident, and family to resolve resident/ family concerns.
- Ensures delivery of compassionate quality care as evidenced by resident/resident representative feedback, observation, and chart review.
- Collaborates with physicians, consultants, community agencies and institutions to improve the quality of services and to resolve identified problems.
- Assists in coordinating services with all other departments to ensure quality care and services are provided.
- Actively participates as a member of the QAA Committee/ in QAPI initiatives.
- Interacts courteously with residents, family members, employees, visitors, vendors, business associates and representatives of government agencies.
- Provides advocacy services to the residents and resident families of the facility in maintaining resident rights, privacy, activities of choice, and individuality.
- Coordinates the referral of services needed for the residents from outside agencies and/or resources not available within the Facility.
- Performs other duties as assigned.
- Knowledge of applicable state and federal guidelines.
- Strong problem-solving skills with ability to multitask and prioritize.
- Excellent communication and interpersonal skills.
- Bachelor's degree in Social Work or related field.
- MSW preferred.
- Familiarity with the geriatric population, the medical setting and community resources.
- One year of experience in a skilled nursing facility or similar setting.
- Participates in in-service education and competency-based activities as indicated
- Requires attendance of staff and team meetings
- Prolonged periods of sitting at a desk and working on a computer
- Must be able to lift up to 15lbs at times
