Job SummaryThe Care Coordinator II for the Reinvesting in Youth Program is responsible for the delivery of coordinated care to meet the needs of youth and their families at risk of or actively involved in the Juvenile Justice System. This position works within a team based setting to assure overall needs of the client are met in support of preventing, reducing, or eliminating involvement with the Juvenile Justice System.Essential Job functionsDeliver coordinated care to meet the needs of youth and their families to prevent, reduce or eliminate involvement in the Juvenile Justice System.Assess child/youth/family's needs and strengths while continuing to support development of the individual service plan in collaboration with the child/youth/family and multi-disciplinary team (e.g. with the Community Asset Navigator, Family Peer Advocate, community partners, and etc.).Provide psychoeducational training for children/youth/families and groups in a variety of settings based on needs of children/youth/families and groups.Implement strength-based service plan for each client including community based visits/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results, and communicating with the multi-disciplinary treatment team.Assess and support children/youth/families to achieve positive and successful outcomes and routinely successfully uses problem-solving skills with self and others utilizing experience, education, age-specific knowledge and cultural competence in order to.Develop, maintain and update all required documentation in a timely manner while utilizing effective written and oral communication.Provide or arrange for transportation of children/youth/families to and from medical, behavioral health, or other treatment related appointments as needed.While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.Education & ExperienceHigh school diploma or GED required.Minimum 1 year of experience in related field required.SPECIAL REQUIREMENTSUnrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.$21.40 Minimum pay rate, $30.50 Maximum pay rate, based on experience.Knowledge, Skills & AbilitiesIn addition to demonstrating the Hillside Professional Competencies of Communicates Effectively, Personal Excellence, Cultural Competence, Builds and Leverages Relationships, and Optimizes Decision Making, the following occupational competencies must be demonstrated:Uphold the highest standards for ethical and professional conduct at all timesKnowledge of all federal, state, and local statutes, regulatory agency standards and Hillside policies.Knowledge of community resources that support youth at risk of entering the juvenile justice systemAbility to engage in a culturally competent wayAbility to de-escalate and manage crisis situations both in-person and by phoneAbility to deliver psychoeducational trainingAbility to advocate for families and youth in a variety of settingsPhysical Demands & Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
