JOB SUMMARY
The Medical Assistant, under the direct supervision of the Registered Nurse and indirect supervision of the Clinical Specialist, functions as a member of the Interdisciplinary Team, participating in and providing input in each individual's Individual Service Plan (ISP). The Registered Nurse and Medical Assistant, in collaboration with the LCX, ensure the medical aspects of each ISP are carried out through coordinated paperwork, communication, and direct services. They promote individual inclusion, independence, and productivity while advocating for personal interests.
JOB FUNCTIONS:
Regulatory and Corporate Compliance

  • Ensure compliance with regulations, agency policies and procedures.
  • Ensure required documentation is complete, accurate and legible.
  • Ensure the agency's incident and compliance management process is followed.
  • Ensure Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures.
  • Maintains confidentiality of Individual information in accordance with the requirements of NYS Mental Hygiene law and the Agency's HIPAA Privacy Policies and Procedures
Financial Management
  • Assist with ensuring adherence to the annual budget.
  • Assist accurate management of individual's funds for medical needs
  • Assist with purchasing and ensuring adequate supply and quality of medical supplies and over the counter medications.
Person Centered Services
  • Provide services to individuals as planned in their Individual Service Plan (ISP), which includes the person's Individual Protective Oversight Plan, Residential Habilitation Plan and Support Plans and ensure all related documentation is completed.
  • Advocate for individuals by assisting them with their circle of supports with being active in the decisions about their medical care and treatment.
  • Provide person directed services, including residential habilitative services and assistance in personal skill areas.
  • Primary responsibility for coordinating medical paperwork including logging of medications and treatments, preparing and gathering forms such as lab work and test results as needed for medical appointments and assisting the RN with the completion of maintaining and filing of medical files.
  • Track and monitor labs, clinical services, diagnostic testing, and medical appointments.
  • Respect the individual's right to privacy and confidentiality by appropriate use of written and verbal communication.
  • Must be familiar with each Individual's medical history and medication allergies. Assists RN in keeping direct care staff informed of Individuals health status.
  • Work collaboratively with others to ensure the individual's health and wellbeing.
  • Ensure emergency medical procedures are followed and consult with the medical chain of command when necessary.
  • Complete weekly order refills, report new or changed scripts and inform RN of all discontinued medications. Orders medical and treatment supplies as needed.
  • Provide services and follow individual's Plan of Nursing Services (PONS).
  • Report and document all individual medical concerns to the appropriate personnel.
  • Schedules and prepare necessary and required paperwork for dental and medical related appointments.
  • Assist in preparing individuals for medical appointments. Transport and attend health and medical appointments.
  • Complete and maintain required medical documentation and ensure medical follow up is completed in an accurate and timely manner.
  • Assure that associates are aware of any medication or treatment changes or medical conditions and document information on appropriate record.
  • Collaborate with the Registered Nurse to coordinate referral of medical services for individuals.
  • Assist Registered Nurse with communicating with family members, Health care providers and LCX.
  • Attends to Individual's medical needs by passing medications, administering treatment and monitoring appliances such as leg splints, braces, etc.
Program/Service Management
  • Maintain an environment in the home which encourages and facilitates optimal personal development.
  • Complete assigned daily responsibilities.
  • Complete documentation, filing and general office maintenance and other routine operational reports, charge reports and any other paperwork as assigned.
  • Assist in services and support opportunities that provide quality of life choices to individuals.
  • Attends staff meetings and life plan meetings and assists RN in reviewing and modifying Individual Program Plans.
Physical Plant and Safety
  • Ensure the site is a safe, clean environment and immediately address areas of concern.
  • Ensure medical equipment and supplies are stored and cleaned properly.
  • Maintain knowledge of fire evacuation plan and ensure plan is followed.
  • Maintain an environment that encourages and facilitates optimal personal development.
  • Practice standard precautions and other appropriate infection control practices.
  • Ensure any vehicles that you use are maintained per agency policy.
Contacts with Others
  • Develop and maintain positive customer relationships both internal and external. External customers include family, service providers and local businesses.
  • Maintain effective working relationships with service providers, community resources and families. Ensure concerns are addressed timely and in a professional manner.
  • Ensure that internal and external written and verbal communications regarding the agency are positive and reflect our mission, vision and values and respect the individuals' right to privacy and confidentiality.
  • Communicates with IRA and day program staff regarding medical issues and appointments, supplies them with medications.
Additional Functions
  • Participate in agency committees, initiatives, projects, and trainings.
  • Assist in coverage at the home as schedule permits and perform assigned duties.
  • Maintain certifications and annual training updates, as required.
  • May be required to use your private vehicle to transport the people we support when an agency vehicle is not available.

  • The person in this position must be able to operate wheelchair vans, if applicable at site.
  • The person in this position must be able to work regularly scheduled hours as well as additional hours as assigned, including overtime, nights, and weekends.
EDUCATION AND EXPERIENCE:
  • High School diploma or equivalent required.
  • Two years' experience working with people with developmental disabilities preferred.
  • CPR/First Aid/AED and SCIP Certification within the first six months of hire and recert when required.
  • Medication Administration Certification required within 3 months of accepting position or as soon as class is offered.
ADDITIONAL QUALIFICATIONS/REQUIREMENTS:
Where position requires driving:
  • Valid driver's license for the state in which the person resides.
  • Acceptable safe driving record.
Where position requires driving individuals served by the Agency:
  • Motor vehicle with up-to-date inspection, that is registered and insured.
  • Will be required to enroll in LENS (Licensed Event Notification System).

AUTHORITY AND ACCOUNTABILITY:
People: This position has no direct authority in relation to people within this role and does not supervise anyone.
Operations: This position operates under the general supervision of the Registered Nurse is responsible for dental, health and medical related appointments and following standard procedures.
Budget: This position must adhere to expense guidelines and programs.
