We value work-life balance and offer an extensive benefit package.We are currently hiring a Law Enforcement Assisted Diversion (LEAD) Case Manager , to work in our Care Coordination Services Agency. As the LEAD Case Manager , you would be responsible for:Make initial contact with eligible participants at the point of diversion or social contact referralUse provided written and electronic tools to fully assess participants’ medical, mental health, substance abuse, legal, and psychosocial needsDevelop a care plan through case conferencing/consultation with LEAD partners and all appropriate providers to guide all service delivery and achievement of desire outcomesMaintain professional relationships with the Albany Police Department, LEAD partners, service providers, and referral sourcesMaintain accurate case records and documentationLEAD Case Manager Minimum Salary: $21.86 per hourLEAD Case Manager Maximum Salary: $21.86 per hourLEAD Case Manager Schedule: Monday – Friday, 8:30am – 4:30pmQualifications:Associates Degree1-3 years of experience in mental health, substance use, or human servicesEducation requirements may be waived by the agency if a candidate demonstrates more significant experience (five or more years) in case management, along with significant continuing education/training hours during previous employmentDeep or personal understanding of substance use disorder, mental illness, and harm reduction strategies (preferred)Experience or familiarity with community and human services dynamic (preferred)Demonstrated ability to overcome adversity (preferred)Empathy developed through life experiences and life interruptions, managing emotions, and navigating systems (preferred)Benefits:Health/Dental/Vision/Life InsuranceRetirement planning optionsEmployee Assistance ProgramGenerous Time Off (as an eligible employee, you will accrue up to 23 days off in your first year, based on your standard work day)Employee Discounts (movie tickets, hotels, flights, amusement parks, etc.)Catholic Charities is an Equal Opportunity Employer.Applicants have rights under Federal Employment Law:Family and Medical Leave Act (FMLA)Equal Employment Opportunity (EEO) Know Your RightsEmployee Polygraph Protection Act (EPPA)Requirements
