What You Will Do to Change Lives
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time; AM 8:30 am - 5:00 pm; Monday - Friday Sign on bonus $ 5000
Expected starting wage is $21. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
* High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
* Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
* Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

Telecare TAY ROC provides case management services to adults, aged 16 to 24, who utilize emergency rooms, inpatient settings, and crisis services to regularly manage their mental health and substance use needs. Through a "whatever it takes" outreach and engagement approach, these programs help reduce the need for inpatient and emergency room care while improving mental health outcomes for the members served by connecting members to supports and resources available within their community. These programs work specifically with Alameda County residents who are not yet connected to county healthcare services. Program staff will provide linkages, supports, and resources to help community members remain in the least-restrictive, most self-sufficient, and recovery-oriented settings possible. The goals of these programs include reducing inpatient and emergency room care and improving mental health outcomes in Alameda County.
