We are building a new team and expanding our services.
Now hiring for 4-day work week Monday- Thursday. You will work 36 hours, but we will pay you for 40 hours.
Assertive Community Treatment (ACT) Housing Specialist Position Summary:
The position is part of an interdisciplinary team that provides quality evidenced based practices to clients living in the community struggling with behavioral health issues. The ACT Housing Specialist will plan and implement housing services for clients who have mental illness and substance abuse disorders.
Job Responsibilities:
  • Attend agency housing meetings weekly, community housing meetings, and establish relationships with community shelters.
  • Participate in developing person-centered treatment plans, monitoring, and documenting progress.
  • Provide case management, individual supportive counseling, and assist in symptom management.
  • Know ACT evidence-based practice (EBP) standards and provide high quality treatment.
  • Coordinate treatment and stabilization with identified natural supports and with community service providers.
Qualifications, Education and Experience:
  • Minimum education, high school diploma or equivalent.
  • Current CPR and First Aid certification
  • Valid full driver's license and safe driving record.
  • Excellent verbal and written communication skills.
  • Experience finding and maintaining housing for clients.
  • Experience with getting benefits for clients.
