Job Summary:
The Providence Center DOC Peer Support Specialist/Discharge Planner provides peer support and discharge planning to inmates at the ACI with substance use disorders.
Duties and Responsibilities:
Facilitate recovery peer support groups
Provide individual recovery coaching
Assist inmates with aftercare planning - making appropriate referrals related to substance abuse services in the community such as substance use disorder treatment, recovery housing, and peer support services.
Involve clients, DOC, and other agencies when appropriate in discharge planning.
Co-facilitate a group at Anchor Providence for people returning to the community after incarceration.
Collaborate with TPC substance abuse therapists, DOC's transitional care coordinators, MAT providers, and other specialized discharge planners within DOC.
Build and maintain collaborative working relationships with community agencies.
Report security concerns according to DOC policies/procedures.
Complete record-keeping in accordance with TPC and DOC standards.
Use DOC's EMR to complete documentation including progress notes.
Document discharge plans in DOC's RE-Entry Services software.
Establish and maintain productive and effective communications with clients, TPC, and DOC staff.
Work in cooperation with co-workers and supervisory staff.
Accept and utilize feedback and supervision.
Attend trainings, staff meetings, case presentations, conferences, etc.
Serve on appropriate Center or DOC committees.
Complete other responsibilities as assigned.
Requirements:
Being a person in recovery (at least 2 years) is required for this position.
At least 1-year experience working in the human services field.
Must have knowledge of substance use disorders, community resources, and criminogenic risk factors.
Valid driver's license and insured auto.
Must have or be willing to pursue CPRS (certified peer support specialist).
Hours are M-F, primarily days, but availability to work 1 day a week from 11:30 am - 7:30 pm.
