**Experience:** Minimum of 1-2 years in hospital pharmacy practice or completion of a pharmacy practice residency in a hospital setting is preferred
**License:** State Pharmacist License in good standing
**Certifications:** None
**Must-Have:**
- Pharmacy Degree (Bachelor's or PharmD) from an accredited school of pharmacy
- State Pharmacist License in good standing
**Job Description:**
- Interpret and evaluate provider orders to ensure safe, rational, and cost-effective drug therapy for patients
- Ensure accuracy in drug dispensing and monitor drug therapy
- Effectively communicate with providers involved in patient care
- Participate in ensuring adherence to drug use policies
- Provide pharmacotherapy education and information for physicians, nurses, and patients
- Responsible for clinical projects and participate in committee work as assigned
- Utilize computerized charting and Meditech systems
