The Housing Case Manager will provide case management services to households in housing programs. About the role: Conduct weekly case meetings with those who are qualified and participating in the housing programs including one in-home a month to complete home inspection Conduct regular reviews and annual requalification case meetings as required by program or funding source Utilize the Wisconsin HMIS system to record client case management documentation Maintain complete and confidential files of all clients\' in accordance with The Salvation Army practices Develop and implement activities to help increase client sufficiency Develop and maintain referral information to assist clients Screen clients for possible admittance to housing programs using prioritization tool and other applicable assessments Coordinate with the The Salvation Army Housing Quality Standards (HQS) inspector to ensure that identified housing for clients meet requirements. Work with Housing Coordinator in regard to tracking all expenditures and balances of grants related to the Housing Program Promote and provide case management for POH families through the Rental Assistance Programs. Implement the seasonal Toy Program in collaboration with partner organizations Participate in workshops, seminars, and other meetings regarding Social Services Be a presenter/representative of the organization in the community as assigned by Director Education: Bachelor\'s degree in social service field Experience: Two years casework experience Certifications: Valid Wisconsin driver\'s license with clearance to drive by TSA insurance.
