Join our team in Buffalo as a full-time Health Educator! This position provides oral health education to children and families in our clinics and provides outreach education and oral hygiene supplies in a wide variety of community locations. In addition, this role provides care coordination to families for new patient registration and scheduling of appointments. Who We Are Community Dental Care is Minnesota's largest non-profit dental organization, providing comprehensive dental care to people of all ages across five clinics in the Twin Cities, Rochester and Buffalo. Health Educator Responsibilities: - Provide one-on-one education and motivational behavior change coaching to parents, pregnant mothers and children regarding good oral hygiene practices and basic nutrition in our Buffalo clinic. - Enroll children 0-12 and pregnant mothers in the Program to Improve Community Oral Health (PICOH) program. - Provide phone call and in person follow-up coaching and education to program participants. - Send email/mail and text correspondence to families such as appointment reminders and Cavity Free Kids notices. - Make phone calls to schedule patients encountered in the clinic or at outreach settings for follow up appointments in the clinic. Provide follow-up support and phone calls for patients with missed appointments. - Collaborate with community partners to coordinate and deliver outreach education and awareness raising at a variety of community locations, including schools, health fairs, community events, classroom education/presentations, etc. - Initiate and register patient charts and document chart notes in open dental electronic health records. - Collect and maintain data on services provided, number of patients served in-clinic and via outreach, and other relevant information for grant and evaluation purposes.
