Job Requirements / Description
Assists supportive housing clients to maintain their housing and increase their self-sufficiency through linkages to resources and the attainment of established goals. In addition to maintaining a caseload, assists management with all audits and the activities of case managers providing assistance, training, and guidance.
MINIMUM QUALIFICATIONS
- Bachelor’s degree plus two (2) years experience in social services (case management experience preferred).
- Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred.
- An automobile, valid Driver’s License, and vehicle insurance coverage are required.
- Occasional late or weekend hours may be required.
RESPONSIBILITIES
- Provides training to new and existing case managers.
- Conducts monthly chart reviews of case managers’ charts.
- Completes monthly reports as requested by Supervisor.
- Becomes a user with HMIS.
- Conducts a minimum of one monthly face-to-face contact per client.
- Provides a minimum of one monthly home visit per client or as needed.
- Completes chronological progress notes within twelve hours of contact with client or collateral encounter.
- All progress notes must be completed in DAP format and signed and dated with appropriate credentials.
- Develops initial service plan specific and tailored to the client needs identified during initial intake assessment/screening process.
- Revises or updates service plans every six months or based upon the client level of need.
- Assists no-income clients to identify appropriate resources to obtain entitlement(s) within six months of admission to the housing program.
- Completes referral documentation and informed consent for supportive services.
- Routinely assesses strengths and barriers to income, housing stability, and self-sufficiency (including health, substance use, and mental health issues), and provides appropriate referrals to supportive services in a timely manner.
- Provides advocacy, support, and linkages/referrals to community resources.
- Conducts follow-up on all referrals and documents referral outcomes.
- Updates client files to ensure a record of all client level activities.
- Enters all client level activities in Client Track within twelve hours of encounter.
- Completes monthly statistical report and submits to immediate Supervisor by the tenth business day of each month.
- Participates in all mandatory external and internal meetings or trainings.
- Adheres to the Agency’s code of ethics and HIPAA Regulations for confidentiality.
- Performs other duties as required or assigned.
