PT Case Manager @ CH Home Manitowoc - Home Care PT

JOB DESCRIPTION

CANDIDATE MUST HAVE:
- 3 years PT experience
- active PT license (WI)
- active BLS (AHA)
- valid Driver's License (DMV)
Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
Support Team: Manager/Supervisor, Lead, Scheduler
Technology/Equipment: EPIC/Computer charting, Laptop, Sapphire IV pumps
Any scrub color acceptable (must be professional).
Traveler must provide own transportation to/from all patient visits. Reimbursement available (restrictions apply).
