Position Purpose:
The PEDS Patient Navigator/Health Educator will collaborate closely with healthcare teams, resource providers, and key stakeholders to improve coordination of patient care. This role focuses on overcoming barriers to timely vaccinations for preventable diseases and emphasizing the importance of staying current with well-child visits and preventive care. Additionally, the PEDS Patient Navigator/Health Educator will lead skill-building groups for parents and caregivers of young children, focusing on developmental milestones and supporting children's health, growth, and development. The Navigator will assist clients and family members by identifying needs, connecting them to resources, and guiding them through the administrative systems and care continuum. This role also involves participating in educational programs, increasing awareness, conducting in-service training for clinical staff, and conducting outreach to patients and families.
Principal Responsibilities:
  1. Engage clients to raise awareness about the importance of timely childhood vaccinations.
  2. Identify patients with social determinants of health (SDOH) factors such as housing, food, and transportation insecurities through JWCH's Registration Forms and refer them to case managers, benefits enrollment, transportation services, food pantries, etc., as appropriate.
  3. Assist with scheduling patients due for immunizations, well-child visits, and/or lead screenings, and track completion rates for these services.
  4. Provide targeted education to parents and caregivers about the importance of timely immunizations, using in-person interactions, phone calls, electronic communication, mail, etc.
  5. Track and reconcile immunization data received from external sources and enter it into the CAIR registry.
  6. Generate PEDS reports to monitor progress by clinic.
  7. Provide educational in-service training to clinical support staff on the importance of timely vaccinations and developmental milestones.
  8. Coordinate with clinical care teams to address any medical barriers identified.
  9. Assist clinics in standardizing pediatric workflows.
  10. Support clients from diverse cultural backgrounds in understanding the healthcare system.
  11. Attend general staff meetings and community outreach screening events.
  12. Perform other relevant tasks as assigned.
Requirements:
  1. Bachelor's degree in Public Health, Nursing, Social Work, or a related field. An LVN or RN background is also acceptable in lieu of a degree.
  2. Minimum of two years of experience in pediatrics, public health, or a related field.
  3. Proven experience in case management or patient navigation preferred.
  4. Strong understanding of Social Determinants of Health (SDOH) and their impact on patient outcomes.
  5. Excellent verbal and written communication skills, with the ability to engage diverse audiences.
  6. Proficient in using electronic health records (EHR) and health information systems.
  7. Ability to work effectively with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  8. Sensitivity to the needs of underserved populations.
  9. Familiarity with childhood vaccination schedules and preventive care guidelines.
  10. Proficiency in English and Spanish is required.
  11. Valid California Driver's License and willingness to travel between various sites as needed.
