Find your purpose working as a Social Worker with CentraCare! As a vital member of our interdisciplinary HomeCare and Hospice team, the Social Worker plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards.

As part of our HomeCare and Hospice team, you will have the opportunity to make a meaningful difference in the lives of those we serve while working in a collaborative and supportive environment. This position will split time between our new Inpatient Hospice Team at the St. Cloud Hospital, Hospice Bereavement program and occasionally support with home care visits around the St. Cloud area.
Schedule
To be eligible for the hiring bonus and relocation assistance, you must be an external candidate who has not worked for CentraCare for at least 9 months at the date of hire and be hired into an eligible full-time or part-time benefited position. Relocation assistance also requires that you currently live more than 50 miles from your primary work location and move within 30 miles of it within 120 days of employment. CentraCare pay promotions to include but not limited to, hiring bonus and relocation assistance may be discontinued at any time due to business needs.

Schedule
- Full-time | 72 hours every two weeks
- Monday-Friday | 8am - 4:30pm
- No weekends or holidays!
- $27.54 or more per hour, pay determined by experience
- $3,000 hiring bonus and $5,000 relocation assistance for eligible candidates

- Bachelor's Degree in Social Work or Bachelor's Degree in Sociology with 5 years of social work experience
- Current Licensed Social Worker (LSW) in Minnesota
- Reliable transportation with a valid Minnesota driver's license, a driving record in good standing, and insurance liability coverage
- Minimum of 1 year of prior relevant clinical experience preferred
