QUALIFICATIONS:

High School Diploma/GED required. Valid driver’s license required with no moving violations in the past 3 years. Must be a Certified Recovery Coach and Peer Support Specialist or obtain certifications within 12 months of hire. Must have thorough knowledge of Native American (Coeur d’Alene Tribe) customs and traditions. Four (4) years of experience in substance use disorder-Addictions, Prevention and or similar capacity that would demonstrate necessary knowledge, skills and abilities preferred. Experience using Microsoft Word and Outlook. Must have excellent communication and interpersonal skills in a mixed culture context. Must have completed a patient transferring course or equivalent level healthcare training with no restrictions or be able to complete course within 6 months of hire; must have current American Red Cross First-Aid card and Basic Life Support (CPR) certification or obtain within 3 months of employment. Must complete defensive driving course within 3 months of employment. Must be able to establish and maintain supportive and professional working relationships with staff, the public, service groups and facility users. Must be able to establish and maintain supportive relationships with Coeur d’Alene Tribal members and Tribal departments. Must be able to pass an Idaho Health and Welfare fingerprint background check. Must meet ID and WA Department of Corrections Volunteer criteria (i.e. No adult felony convictions or misdemeanor drug convictions in the last 3 years, no outstanding warrants or criminal charges, not have been released from a state, local, or out-of-state corrections agency or supervision in the last 12 months, appropriately certified/licensed to provide the professional services engaged to provide, no current or prior relationship with an incarcerated individual at the facility).

  • Sobriety is an essential function of the Community Outreach and Transportation position.

RESPONSIBILITIES:

  • Maintains a positive peer relationship and performs as a team player.
  • Plans and prioritizes to maintain a time and attendance record which complies with Marimn Health policy.
  • Provides excellent internal and external customer service assistance, providing knowledgeable and appropriate information to customers.
  • Works independently in a very detail-oriented manner and meets deadlines.
  • Ability to work with various agencies including but not limited to jails, hospitals, treatment facilities and other various programs.
  • Employee reports to work in a timely manner.
  • Employees utilize breaks and meal periods to care for personal business outside of the work area.
  • Employee completes work assignments in a timely manner and appropriately exits the work area in a timely manner.
  • Employee appropriately utilizes Time and Attendance for clocking in and out and schedules absences in advance when possible.
  • Employee is available for scheduled work shifts regularly and communicates absence to supervisor in a timely manner if unavailable.
  • Employee is flexible in working days and hours to support community outreach events and activities.
  • Recovery coach certified and able to provide services consistent with certification and need and a ble to utilize peer support/recovery coach training to understand recoveree’s level of care.
  • In coordination with the Wellbriety team, develop a monthly and annual calendar of events for Wellbriety participants.
  • Plans, organizes and facilitates events, including ordering all supplies and completing event evaluations.
  • Collaborate with Marimn Health Marketing team to complete activity request forms for each event with all pertinent information to be approved and then shared with necessary departments/social media.
  • Organizes planning committees as necessary and directed by program manager to fulfill Wellbriety activities and events.
  • Conduct participant surveys to evaluate community needs, grant compliance and strategy effectiveness.
  • Identify and expand available, culturally appropriate Behavioral Health Care Prevention, Intervention and Recovery services.
  • Coordinate with local Community partners to improve care coordination and the referral processes.
  • Administer, coordinate and evaluate prevention and recovery related training, education, programming and activities.
  • Collaborate with Wellbriety Team to plan, develop, implement and monitor social media content.
  • Acts as a strong advocate and proponent for the Wellbriety Program.
  • Possess working knowledge and skills of multiple Tribal Best Practices, prevention and recovery models of change such as MAT, stages of change, abstinence, moderation management, White Bison, Healing of the Canoe, Life Skills, faith based, and 12-step.
  • Willing to serve as a manager on duty (MOD) when needed.
  • Maintains data on client participation, community outreach, health fairs, and education events and provides reports quarterly and as requested in alignment with departmental grant reporting timelines.
  • Schedule and coordinate transportation services for eligible Wellbriety recoverees according to the department’s policy.
  • Responsible for transportation in support to the participants goals in Individual Service Plan alignment with treatment plans established with Behavioral Health providers and staff.
  • Maintains and follows established transportation schedule based on recoveree needs, prioritizing transportation to program-related appointments on a scheduled basis.
  • Physically able and willing to assist recoveree/elderly with ambulation, including loading and unloading of non-ambulatory patients.
  • Able to navigate safely in rural and urban traffic and locate referral facilities.
  • Performs pre-trip inspection of vehicle assuring proper working order. Alerts management to any issues or needs promptly.
  • Operates vehicle in a safe and courteous manner with regard to traffic safety regulations and comfort of passengers.
  • Maintains inventory of vehicle supplies and equipment.
  • Advises supervisor of road hazards and inclement weather conditions for consideration of temporary cancellation of transportation services.
  • Maintain timely and accurate record keeping, capturing transport mileage and destination reports in adherence to program policy, ensuring reimbursement and billing standards are met.
  • Ensures servicing of vehicles is completed when required i.e. oil changes/seasonal tire installation/wiper replacement.
  • Available for potential long, off-hours transportation i.e. picking up recoverees from in-patient treatment out of the area. These trips are infrequent and may result in overtime hours.
  • Transports other Marimn Health patients as needed.
  • Ability to utilize peer support/recovery coach training during transport to understand recoveree’s level of care.
  • Utilizes department and organization electronic software including Northpointe and Air Slate systems to document all client face-to-face contacts, collateral contacts, referrals, and telephone calls regarding care.
  • Documents case management activities that meet both clinical and billing criterion and standards and completes all documentation within 24 hours.
  • Ensures that all required documentation and reporting is confidential and complete in a timely, accurate, and efficient manner
  • Performs duties and responsibilities assigned by supervisors in the effort of providing coordinated care to recoveree.
  • Actively participates in organizational and departmental meetings to enhance knowledge of objectives, goals and activities.
  • Actively engages in Quality Improvement activities related to the department.
  • Maintain Chain of Command and uses proper internal channels to communicate and voice issues/concerns.
  • Other duties as assigned.
