Position Summary

The Assistant Director of Care Management is responsible for upholding the mission of Lawndale Christian Health Center, working under the Director of Care Management to provide leadership for care management staff and programs. The Assistant Director is responsible for supporting Care Coordination Managers to equip and co-supervise Care Managers. They collaborate with clinic operations staff to ensure integration of care management within the clinic. The Assistant Director is responsible for delegated oversight of Care Management programs and implementation of new programs related to care coordination.

Principal Duties & Responsibilities:

Program Management and Administration:

  • Supports Director with oversight and administration of federal, state, and private grant-based programming and their deliverables. Works alongside the Development Department on grant-specific requirements and audits.
  • Leads development of new workflows for delegated new projects and programs related to Care management including care coordination, transitions of care, chronic disease education, and other social service support.
  • Collaborate with Health Information and Electronic Medical Record teams to ensure effective and efficient processes to support care managers in tracking their work and to ensure that program performance metrics are adequately reported to external systems for program compliance.
  • Work with external relations manager and Care Management Leadership team in collaborating with partners and funders. Support Care Coordination Managers to participate directly in various committees and workgroups related to different population groups including maternal child health, chronic condition support, and adult complex care management.
  • Leads ongoing coordination efforts with outside partners in care management related activities.

Staff Performance and Operations:

  • Leads CM team rounding support for Care Managers and partners with Care Coordination Managers in providing direct rounding/supervision support for all care management staff at LCHC.
  • Partners with director in ensuring Care Management staff are properly trained, integrated within their care teams, and effective in following CM workflows. Support director in working with CM leadership and other leaders to ensure staff have adequate clinical training to engage and intervene in supporting complex patients.
  • Provides direct supervision of Care Coordination Managers (CCM) and oversight of CCM support of Care Managers and related team members.

Clinic Integration and Implementation:

  • Leads Care Coordination Managers in working with site managers on the care teams to support care manager integration and supports site managers with data and direct feedback to support them managing care managers.
  • Supports Director in management of interventions to reduce the impact of chronic disease for priority areas such as maternal child health, substance use, HIV/AIDS and more.
  • Performing other duties as required to assist LCHC in achieving its mission.

  • Exceptional organizational skills for ensuring the completion of a large volume of work in a systematic manner.
  • Initiative and creativity for problem solving and pro-active improvement of staff engagement and clinic workflows.
  • Capable of exemplifying the values of Lawndale Christian Health Center in all circumstances.
  • Strong time management skills and experience with directly supervising and managing staff.
  • Embraces and personifies the mission of Lawndale Christian Health Center, by performing work in a whole person way, providing for the physical, emotional, and spiritual needs of patients.

Required Knowledge, Experience, or Licensure/Registration:

  • At least 3 years’ experience in community health, care coordination, case management of patients of various risk groups and chronically ill patient groups.
  • Demonstrated managerial experience in a healthcare setting with deep understanding of best practices of collaborative management processes required.
  • Master’s Degree in Social Work or Nursing Degree from an accredited university and Illinois State Licensure preferred.
  • Knowledge of the major functional areas of health systems operations, including all clinical, health support, and administrative departments.
