Job Description:
- Ensure all prescreen forms are completed and attached for submission.
- Attach a resume and license verification for application consideration.
- Conduct a COVID test 72 hours prior and obtain results 48 hours before the start date.
- Complete credentialing checklist including:
- State License Verification
- Professional References (3)
- County and Federal Criminal Background Check (7 years)
- Multi-State Criminal Search (7 years)
- Nationwide Sex Offender Search (7 years)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- Complete a 2-step PPD or Quantiferon, or provide proof of positive PPD and chest X-ray
- SAM Search
- Oregon State Background Check (facility run)
