JOB SUMMARY
The Keys to Change Facilitator/Case Manager position reports to the Keys to Change Program Manager and is accountable for Facilitation and case management for the Keys to Change program in San Joaquin County. This position provides direct services to participants to assist them in reintegration into the community following a period of incarceration, including providing case management services, facilitating Cognitive Behavioral Intervention (CBI) workshops and accurately documenting participants' progress and outcomes.

JOB DUTIES AND RESPONSIBILITIES
  • Work directly with participants, the San Joaquin Probation and Sheriff's Departments, and local service providers to develop reintegration plans following incarceration
  • Conduct pre-release program intake interviews to determine the needed service and support levels related to education, employment, family, recovery, and community
  • Develop individualized reentry and stabilization plans with assigned clients
  • Facilitate daily workshops for eight week CBI Employment cohorts
  • Assist Program Manager with all data reports as needed
  • Develop, revise, and implement curriculum that aligns with the evidence-based practices of the CBI program
  • Develop and facilitate strategy for community education and awareness of the needs of formerly incarcerated clients
  • Network with other reentry programs to referrals to other services as needed
  • Build and maintain relationships within the network of local service providers
  • Make referrals to probation department employment program as needed
  • Utilize Apricot 360 or other client databases to keep accurate and up-to-date records of employers, placements, and other client information
  • Timely and accurately document appropriate areas of participant case files with progress, outcomes, obstacles, and other activities
  • Assist with program orientation as needed
  • Coordinate efforts of internal and external partners to prove case management services to participants
  • As needed, provide counseling and crisis intervention services to program participants
  • As needed, take a lead role in criminal justice and reentry training, planning and implementation initiatives as needed
  • Attend program and agency related meetings and conferences
  • Abide by Five Keys and Probation policies and procedures
  • Other duties as assigned
KNOWLEDGE AND SKILLS
  • Excellent public relations, community organizing, negotiation, mediation, conflict management, and meeting facilitation skills
  • Demonstrated ability to inspire community participation and build coalitions
  • Advanced proficiency in Zoom, Microsoft Office, Google Workspace, and databases, including word processing, email, internet, and spreadsheets
  • Able to maintain strict confidentiality and take all precautions when handling sensitive information
  • Ability to effectively and efficiently work from home and independently, with high levels of self-accountability
EDUCATION AND WORK EXPERIENCE
  • 2 years case management, counseling, and assessment experience
  • Previous re entry experience preferred OR
  • Equivalent combination of education and experience
  • Preference will be given to qualified applicants who are returning citizens
  • Upon hire, successful complete certifications in CBI-Employment and Motivational Interviewing required
PHYSICAL REQUIREMENTS

  • Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
  • Frequent talking and hearing conversations
  • Long periods of walking, sitting at a desk and/or computer station
  • Lifting items that weigh up to 25 lbs.; reaching, bending, carrying and stooping within the daily routine of tasks and activities
ADDITIONAL REQUIREMENTS
This position is contingent upon:
  • Satisfactory results of a Live Scan (criminal background investigation)
  • Successfully obtaining jail clearance for positions working in county jails or probation locations
  • Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable

  • Work environment varies from site to site
  • Some personal safety risk working in underserved communities
