Hazel Park Schools is seeking applicants for the position of Transition Coordinator at the Jardon Vocational Program, for the 2024/25 school year. The school is served by a caring and professional teaching staff providing instruction to approximately 65 students. GOAL: To provide services and resources for students, parents, and staff in the areas of vocational evaluation, transition planning, employment skills development, and when appropriate, employment placement for students.
QUALIFICATIONS:
1. Bachelor's Degree in special education from an accredited institution.
2. A minimum of 3 years of satisfactory teaching experience in special or vocational education.
3. Preferred experience in transition planning, youth employment, and vocational assessment of students with special needs.
4. Requirements as defined in the Michigan Administrative Rules of Special Education and through Oakland Schools Plan for delivery of special education programs and services.
PERFORMANCE RESPONSIBILITIES:
Under general the supervision of the Principal, provides leadership and administration that will motivate instructional and support personnel to strive for superior performance in order to provide the best possible opportunities for student growth and development, both educationally and personally.
1. Provides for the vocational evaluation of secondary students with special needs.
2. Provides written reports of results of vocational evaluations with specific recommendations for transition planning.
3. Provides vocational counseling and assists in student career and transition planning.
4. Provides staff with the resources needed to develop and carry out effective transition plans.
5. Assists students, as needed, in the acquisition of basic employment skills.
6. Responsible for the organization and implementation of school's Work-Based Learning Program (working relationship with businesses and transportation department, ensure safety training and attendance taken at each worksite, WBL placement of students in three rotations a year; complete and file Training Agreements, Bi-Monthly Work Reports and Post-Assessments) and services to students based on educational need at job sites in the community and within the school district.
7. Responsible for scheduling student worksites, paraprofessional schedules for worksites, and schedules for students requiring individual support during the day.
8. Responsible for visiting worksites to ensure employer satisfaction and student participation.
9. Serves as a liaison between community transition agencies and special services staff.
10. Trains and directs paraprofessionals and teachers to support transition programming.
11. Consults with and assists the Building Principal and Director of Students Services.
12. Maintains professional competence through in-services, workshops, and other professional development activities.
13. Attends IEP meetings in the role of Transition Coordinator and participates in the IEP process as appropriate and as required by law.
14. Participates in staff meetings, committees and the sponsorship of pupil activities.
15. Develop and maintain current Student Information Binders (Student Information Sheets, Behavior Plans and Health Plans), distribute information to job coaches.
16. Collaborates with families of graduating students and adult service providers to develop a meaningful transition plan for students after school.
17. Adheres to all applicable laws, policies, and regulations governing the operation of Hazel Park Schools.
18. Other duties as assigned by the Building Principal/Supervisor or Director of Student Services.
EVALUATION: Performance of this job will be evaluated in accordance with the provisions of the Board's policy on evaluation of staff.
Compensation: Accordance with the Salary Schedule - H.P.E.A. Contract
