General Statement of the Job & Essential Functions
GENERAL STATEMENT OF JOB
Under general supervision, assists with crisis intervention, service coordination, screening, and assessments for mentally ill, substance use, and co-occurring children and adolescents and their families for the Children's Community Crisis Stabilization Services (CCCSS). Work involves assisting individuals by identifying individual needs. Employee provides peer to peer supportive counseling regarding recovery and stabilization. Work involves peer support to ensure mental and physical well-being. Employee documents clients' progress and maintains records in accordance with program policies and procedures. This is a grant funded position and continuation is subject to the availability of funds. Employee may transport individuals. Employee reports to the Program Administrator (CCCSS).
ESSENTIAL JOB FUNCTIONS
Provides peer-to-peer supportive counseling to consumers using wrap around service, self-disclosure regarding recovery process and conducts one-on-one counseling. Provides consultation to individuals to assess their social, health, and vocational needs; and assists consumers in obtaining information regarding appropriate community service programs to promote recovery. Assists the Case Manager with providing crisis intervention.
Implements goals and objectives by linking individuals to appropriate services. Assists in the development of treatment plans and determination of consumers' goals. Provides outreach services to locate and re-engage consumers for follow up and outcome data.
Ensures safety of consumers. Consults with staff regarding continuity of individual's care. Transports individuals to and from assigned locations if applicable.
Keep data and outcomes for the grant as requested. Maintains accurate and complete records and files according to program policies and procedures. Participates in staff meetings and team meetings to ensure team approach and a continuum of care. Attends related training/workshops. Perform routine administrative duties as needed for gathering data and statistics.
Performs other related work as required. Individual assignments will be determined by supervisor based on current workload and department needs.

Education, Experience & Special Requirements
EDUCATION & EXPERIENCE
Graduation from a college with an Associate's Degree, in a human services field preferred, and requires 1 to 3 years of related experience or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
SPECIAL REQUIREMENTS
Must possess Red Cross First Aid and CPR certifications or have the ability to obtain certifications within 90 days from date of hire and maintain them as a condition of employment.
A valid driver's license with an acceptable driving record.
An acceptable general background check to include a local, state, and federal criminal history check along with a sex offender registry check.
