Description
The Care Connection Engagement Specialist will be the first line of contact for prospective members connecting with ACA/NY CCO. This role is responsible for collecting and documenting all pertinent demographic information for the prospective member in ACA/NY's enrollment database. The Care Connection Engagement Specialist will serve as a resource in explaining and navigating the eligibility and enrollment process for CCO/HH, and then direct and assign the prospective member to the dedicated regionally based Care Connection Specialist to initiate intake and complete the enrollment process. The Care Connection Engagement Specialist will apply prior experience and knowledge of existing programs to support individuals with developmental disabilities. They will establish relationships with OPWDD Front Door Staff, Local Department of Social Services Staff, and others to assist with outreach efforts. The primary functions of the Care Connection Engagement Specialist will be answering referral calls and emails, collecting demographic information, explaining the process of enrollment and Care Management services, tracking Care Connection Specialist's rosters and distributing referrals accordingly. Administrative tasks will be required and determined based on the needs of the Enrollment department. The Care Connection Engagement Specialist role embodies and represents the philosophy and mission of ACA/NY CCO and is focused on quality customer service within a person-centered approach. This role understands and prioritizes communication and responsiveness to prospective members and referral sources.
ACA/NY is a 501(c)(3) not for profit organization that has been designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. ACA/NY is dedicated to meeting the needs of people with Intellectual and Developmental Disabilities by providing comprehensive care management and coordination of service. ACA/NY supports 25,000+ people in its program with support services spanning New York City, Long Island, and the Lower Hudson Valley.
Duties and Responsibilities :
  • Receiving and processing phone calls (or e-mails) regarding intake and enrollment, as well as general inquiry calls related to CCO services.
  • Discussing, explaining and/or assisting the person/family/advocate with the formal CCO intake and enrollment process.
  • Complies with OPWDD and DOH regulations pertinent to Care Coordination.
  • Maintains all relevant documents in the Person's record, including all phone contacts, correspondence, assessments, and reports, if applicable.
  • Ensures all confidential records are secured and initiate enrollment to ACA/NY CCO.
  • Utilize M-Files and the EHR to support department efforts.
  • Reports to supervisor and/ Care Connection management team the status of referrals; including but not limited to # of people eligible both with and without Medicaid.
  • Establishes and maintains a community network between service providers internal and external to the Agency.
  • Form and maintain working relationships with allOPWDD Regional Offices, with the goal of ensuring that ACA/NY is positioned as a responsive CCO when a referral is needed.
  • Provide referral information for those who require a different service model.
  • Attend department/team meetings, trainings, supervisions, etc. as scheduled and in accordancewith agency practice and policy. Complete all required trainings.
  • Travel throughout the designated service area to meet with people interested inreceiving services as needed.
  • Maintain confidentiality.
  • Perform other duties as assigned.
Qualifications
  • Bachelor's degree preferred.
  • At least 3 to 5 years of related experience, specifically in working with people with intellectual or developmental disabilities.
