LPN

Under RN supervision, provide nursing care needs to participants in PACE center and in community at participants' homes. Oversee med management, participate in planning and implementing nursing care plans, provide teaching to participants and families. Perform nursing care at home and in homes within scope of practice. Collaborate with IDT. M-F 8-4; some weekends and holidays if needed. Provide care in clinic and in participants' homes. Qualifications License LPN in Massachusetts Graduate from an accredited school Min 1 year geriatric experience Self-directed, organized, and flexible
