Position Summary: The Home Health Intake Coordinator oversees the initial intake process for patients referred to home health services. They collaborate with patients and healthcare providers, gathering essential information, coordinating assessments, and ensuring a seamless transition into home-based care. Salary Description
$40.00-$50.00
Position Summary: The Home Health Intake Coordinator oversees the initial intake process for patients referred to home health services. They collaborate with patients and healthcare providers, gathering essential information, coordinating assessments, and ensuring a seamless transition into home-based care. Their role encompasses providing exceptional customer service, evaluating eligibility, scheduling visits, and initiating the care process to facilitate timely and efficient care delivery.
- Receive and process new patient referrals using industry leading EMR system
- Performs accurate and timely data entry
- Assigns new patients to a Case Manager to ensure the admissions visit is completed within 48 hours of referral receipt
- Communicate with various healthcare professionals to provide highest level of care
- Use HIPPA compliant eFax system to receive and process orders for new and current patients
- Perform follow up calls to patients and families to offer clear and supportive communication
- Perform other various coordination duties to support day to day workflow
Schedule Options:
- Any 5 day schedule that include Saturday and Sunday
- Weekdays are in office and weekends are remote
- $40.00-$50.00
- This position is also eligible for monthly productivity bonuses
- Compensation is determined by the following factors; experience, knowledge, skills, location, as well as internal equity and alignment with market data.

Qualifications:
- Two years of administrative experience preferably in a medical setting required
- Direct Intake and/or patient registrations/admissions experience highly desired
