DEPARTMENT: CPSP
REPORTS TO: Associate Director of Perinatal Department
SCHEDULE: Full Time, Monday- Friday; some evenings required
LOCATION: Davis, Woodland and West Sacramento. Travel to all 3 sites required
PAY RANGE:
  • Master's Degree: $32.80/hr.
  • ASW/AMFT/APCC: $71,760.00 to $75,337.60 annually
  • LCSW/LMFT/LPCC: $81,473.60 to $99,590.40 annually

JOB SUMMARY: The Perinatal Social Worker (PN SW) works with a multidisciplinary team which may include midwives, nurses, dieticians, health educators, behavioral health staff, social workers, and physicians to provide direct client services for Perinatal patients in Davis, Woodland, and West Sacramento clinic locations. Our client population is low income and multicultural. CommuniCare+OLE's mission is to provide non-judgmental health care, education, and referrals to improve the health and wellness of clients and families served.
EDUCATION, EXPERIENCE, TRAINING
  1. Master's degree in Social Work, Psychology, or Marriage, Family and Child Counseling required.
  2. One year's experience in maternal and child health is preferred, with an ability to provide social work services to low-income clients including short-term interventions for stabilization and referrals.
  3. Bilingual Spanish/ English preferred.
  4. Experience with LGBTQ, multicultural populations and substance use assessment and intervention skills are preferred with knowledge of local resources.

DUTIES AND RESPONSIBILITIES
1.
Oversee psychosocial assessments and referrals for perinatal patients enrolled in the Comprehensive Perinatal Services Program (CPSP).
2.
Complete risk status assessments and comprehensive social work care plans for perinatal patients.
3.
Interview perinatal patients with psychosocial risk factors and assess for follow up services and referrals each trimester or more frequently as indicated.
4.
Participate in group prenatal and postpartum visits and health education sessions as needed.
5.
Link clients to appropriate services including substance use disorder treatment and other programs within CommuniCare+OLE and the outside community.
6.
Serve as a communication liaison between CommuniCare+OLE's Perinatal and Behavioral Health staff working within programs and grants that involve perinatal patients and psychosocial assessments/care.
7.
Review and maintain client referral materials and a resource list and make these accessible to colleagues across departments.
8.
Maintain and update written protocols for the delivery of perinatal psychosocial support services in coordination with CPSP Manager.
9.
Attend meetings as required to collaborate with other program and agency staff.
10.
Monitor patients with high-risk psychosocial status with the Lead Midwives at each clinic site.
11.
Coordinate psychosocial care plans with social workers at Sutter Davis Hospital.
12.
Assess and improve CPSP social work quality and outcomes.
13.
Perform or supervise accurate data input and reporting.
14.
Attend and provide updates at monthly CommuniCare+OLE Perinatal staff meetings.
15.
Attend and provide training as needed to assist with CPSP compliance.
16.
Attend meetings for community engagement outside Communicare+OLE as directed.
