Remote Behavioral Health Case Worker - Serious Mental Illness

Responsibilities:

1. Manage and coordinate care for Members with behavioral health conditions and acute, chronic, and medically complex needs.
2. Act as the primary point of contact for the care team, guiding members in achieving optimal health.
3. Conduct face-to-face visits to provide education and assessment at members' residences, provider's offices, hospitals, and community locations.
4. Support members during transitions of care, evaluate and update care plans, and maintain accurate documentation of member interactions.
5. Establish points of contact to collaborate with identified community, medical, and/or behavioral health teams.

Education/Experience:

• Bachelor's Degree in Human Services or related field. Masters prepared will be given preferred consideration.
• 2 years of professional experience working directly with SMI members.
• Proficiency in MS Office, internet applications, electronic medical record and documentation programs.
• Ability to manage a large caseload, strong documentation and record-keeping skills.
• Travel is required, must possess reliable transportation, a valid driver's license, and current auto insurance.
