SocialWorker - Delta/Montrose
Full-time - 40hrs/week
Find your lifes work at HopeWest as Social Worker supporting our patients andfamilies in Delta & Montrose Counties. This role will work 20hrs/week ineach location which offers a unique opportunity for flexible scheduling, havinga "mobile office", being out on the road and seeing ourpatients/families in their personal living environments in these beautifulrural communities.
You will be part of an interdisciplinary careteam of social workers, chaplains, nurses, physicians, and certified nursingassistants all working together to improve peoples everyday lives when theyneed help the most. You will be an advocate by providing education andresources to patients and their families.
The Social Worker will work with an exemplaryand robust interdisciplinary team focused on providing quality care to patientsin the community through education, resources, and care that honors the goalsand wishes of each person. While the Social Worker does provide emotionalsupport, the bulk of this role is not therapy/counseling.

Who Should Apply?
- Social Workers with a minimum of one year experience in healthcare.
- Experience working (paid or volunteer) with the elderly preferred.
- Excellent interpersonal and communication skills.
- Applicants who live in and can commute between Delta & Montrose, CO & enjoy drive time.
- MSW with Colorado Social Work licensure preferred, BSW required
- Minimum of one (1) year supervised experienced in the health care field or during a supervised internship.
- Participates in the on-call, holiday, weekend, and after-hours rotation.

- Premium pay for Licensed Clinical Social Worker and/or bilingual fluency
- Mileage reimbursement of 60cents/mile for work related driving
