Purpose of Role
The Case Manager ensures follow-up with a client's needs, objectives and goals, working in conjunction with the clinical team to address any case management needs including, but not limited to discharge plans that ensure long term sobriety and sound mental health and intake processing.
Core Responsibilities:
  • Documents referrals, family, employer and other contacts (during treatment, at intake, and at discharge) according to requirements set forth by clinical staff.
  • Complete intakes for new admissions outlining service needs, strengths, abilities, and treatment preferences and starting discharge process at admission which each individual served.
  • Maintains awareness of client needs upon entry, utilizes such information to develop appropriate discharge plans, and implements plans accordingly.
  • Monitor, evaluate, and record client progress with respect to discharge goals.
  • Coordinates comprehensive discharge plan with needed resources in the community to ensure seamless community reintegration.
  • Oversees all aspects of case management, working closely with clinical staff, utilization management and alumni coordinators.
  • Help client with financial responsibilities and ensures follow up of those responsibilities. Including budgeting for next level of care.
  • Assists with FMLA and short term disability.
  • Understands the need for appropriate client/staff boundaries - discontinues communication with clients upon discharge. Informs supervisor of client contact after discharge.
  • Meets quality expectations of accuracy, documentation and record-keeping.
  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, clients, visitors, families and referral sources.
  • Performs office duties as required including answering phones, faxing, typing, filing and other assignments as needed by administrative staff.
  • Completes groups as requested to address aftercare needs and life skills necessary to move into next level of care following discharge.
  • Updates job knowledge by participating in educational opportunities and completing 20 annual Continuing Education Units per FAC 65D-30.
  • Understand and follow all policies and procedures set forth by the Clinical Director, Executive Team, and President.
  • Adhere to all company policies and safety rules, guidelines & procedures.
Professional Experience, Essential Skills, and Knowledge
  • At least two years of Substance Abuse and Mental Health Treatment Experience
  • All employees may be required to work before, during and/or immediately after a natural or man-made disaster
  • Understands the principles of psychiatric and addictions treatment
  • Ability to prioritize workload and work independently, and must be thorough and communicate effectively
  • Computer knowledge and use of computer systems a must
  • Ability to communicate effectively with staff, management, and clients
  • Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws
  • Is knowledgeable of Clients Rights and treats all clients with dignity and respect in a consistent caring manner; adheres to federal confidentiality standards (HIPAA)
  • Must be reliable and consistent with attendance and punctuality to work
