Job Overview
Success Coach will recruit and register families, complete intakes, create assessments, and set individualized goals for youth and families. They conduct routine plan reviews to ensure plans meet participant needs, facilitate service referrals, and track access and follow-up. Success Coach will collaborate with the Raising the BAR Program Manager and community partners to provide client-centered care coordination. Success Coach will conduct parenting workshops, meet one-on-one with families, and make home visits if needed. Success Coach will document service activities in detailed case notes, conduct satisfaction surveys, and work to expand the referral base. A bachelor's degree in social work or a related field and at least one year of relevant experience are required. Knowledge of the community and social issues is recommended. Additional experience in juvenile assessment, counseling, or family intervention can substitute for educational requirements.

  • Maintain a daily journal of activities completed in the assigned Micro-Target Area where assigned: Miami Dade or Broward County.
  • Maintain a log of daily activities and contacts, including client files, document referrals, services, and
  • Ensure accurate and timely reporting of activities for assigned Identify progress toward meeting goals and objectives. Submit data and narratives for weekly, monthly, and quarterly reports.

  • Ability to respond to questions from the general public, law enforcement, neighborhood residents, youth, and staff of various community-based organizations.

Required Qualifications:
  • Bachelor's degree in criminal justice, Social Work, Human Services or related field with two years or more of experience in the field.
  • English language is required (can read, write and speak fluently).

Community Engagement
  • Encourage Civic Engagement by bringing government to The Success Coaches will go out to Raising the Bar Micro-target area prioritizing Hotspot Areas 1, 2 and 3. This includes bus stops, businesses, faith-based organizations, and landmarks where the public gathers (Post COVID-19). There is a three-step process involved:
    • Inform people about services, Family & Neighborhood Empowerment
    • Ask people what they need to see in their neighborhood and assess household and individual needs.
    • Identify community resources and services, provide referrals, make services connections, and perform follow-up.

This position is regularly required to sit, stand, walk, speak, and hear. The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen.
