Job Description:
Overall responsibilities include but are not limited to:
Completes biopsychosocial assessments on all patients
- Makes referrals to appropriate organizations
- Provides and assists patients with required applications to apply for patient assistance programs, as well as organize, coordinate, and follow up on patient applications
- Serves as a patient advocate to access federal, state, and local resources identified to address financial and/or medical needs
- Documents all financial assessments, interventions, outcomes, referrals, and services provided
- Facilitates conversation surrounding advance care planning and advance directives, end of life issues, nursing homes/assisted living transitions, and hospice care.
- Communicates and collaborates with the pharmacy, reimbursement, and nursing departments to minimize duplication of effort/services and ensure a seamless coordination of services
- Communicates and interacts with peers and other departments professionally
- Interacts effectively with patients/family members and demonstrates sensitivity in regard to cultural diversity, age specific needs, and confidentiality
- Follows up on distress screenings, providing appropriate emotional support and referring to ancillary resources as needed
- Collaborates with care team to evaluate patient's psycho and social needs and refer patients to external providers when necessary and ensure proper referral information is provided
- Responds to requests from care team and physicians to see patients who are in immediate need
- Establishes and nurtures external relationships with providers that support patients, such as lawyers, mental health services, and community organizations
- Attends networking, training, and education events to help expand knowledge and networks of resources
- Other duties as assigned.
- Master's degree in social work with Pennsylvania license or ability to obtain licensure with in 6 months of hire.
- Oncology experience preferred.
- 2-3 years relevant experience
- Excellent verbal and written communication skills
