Identifies and confers with appropriate resources regarding patient caredecisions.Planning - Develops a plan that prescribesstrategies to attain expected, measurable outcomes.Outcomes Identification - Identifies expectedoutcomes for a plan individualized to the patient or situation.Knowledge - In collaboration withLeader/Director, actively pursues required knowledge and skills through orientationactivities specific to the unit, unit based in-service programs, readingcurrent literature and seeking new clinical learning opportunities.Implementation - Implements care and supervisescare delivered by other team members in alignment with the plan of care andapproved safety and infection control standards.Evaluation- Evaluates progress toward attainmentof goals and outcomes. Reports data and outcomes to as appropriate to physicianand/or other disciplines.Diagnosis - Analyzes assessment data todetermine actual or potential diagnosis, problems, and issuesProvides rationale for patient care decisionsbased on standards of care specific to departmentCollaborates with physicians and other membersof the health care team to address issues related to patients through theprocess of mutual information sharing, discussion, and decision making.Participates in the ongoing variance monitoringof clinical pathwaysAssists with GI diagnostic and therapeuticprocedures and performing cleaning of scopesPerforms and assists with procedures including,but not limited to conscious sedation, pain management, IV therapy, blood andbyproduct administration, bronchoscopy, minor surgeryTakes call after hours as assignedEducates patients and families appropriately fortheir procedures and treatmenConsistently processes patient charges forprocedures and supplies correctlyConsistently documents care and service inaccordance with current Plan for Provision of Care, systems (i.e. Meditech) andprotocols.Handles patient phone calls for patienteducation and/or reassurance needsAssessment- Collects pertinent data andinformation relative to the patient's health or the situation. Recognizes normaland abnormal findings when gathering data.Coordination of Care- Plans and evaluates carein collaboration with appropriate disciplines for assigned patients.
